Someone with a mental disorder is caused by a bio-psycho-social disorder. People with mental disorders in the world are expected to increase along with the dynamics of people's lives. One mental disorder that often arises is hallucinations. Hallucinations are the loss of human ability to distinguish internal stimuli (thoughts) and external stimuli (the outside world). Clients give perceptions or opinions about the environment without objects or real stimuli. Hallucinations undergo sensory changes in perception: feeling a false sensory in the form of sound, vision, taste, touch or smell. Of all patients with schizophrenia, 25 people experienced hallucinations, which were around 40% or 11 patients. Of the 11 hallucinatory patients who experience auditory hallucinations of about 90% or 10 patients in RSSA. The purpose of writing this report is to get an idea of the nursing process for clients with hallucinations: hearing. Includes: assessment, diagnosis, intervention, implementation and evaluation of nursing.
